Physiological Evaluation Colorado Clients Call or email me to discuss what course to take in your evaluation process. You can ask your primary physician to run the profile below under your insurance or if you have recent tests we can use those. I can order tests for you if you do not have insurance or choose not to use it, and would need payment up front for the testing we choose as appropriate for you. The history form below is also needed for our evaluation. If you choose to, we can do an evaluation without the blood or urine testing and I can tell you more about that when we talk. Call me at (303)-697-1736 or email me with questions.Out-Of-State Clients If you live in a state other than Colorado, the profile form below can be taken to your primary physician for a blood draw and urine analysis and run under your insurance with his or her approval. The results need to be faxed or mailed to me and you may need to sign a release for transfer of results to me. If there is a LabCorp in your area and you do not have or want to use insurance, I can order testing for you and would need to be paid ahead to order the tests appropriate for you. When I have your history form and lab results, I will call you to set up our consultation appointment. The consultation will take about one hour, and we will go over all of your tests to determine what is going on with your endocrine, immune, digestive, eliminative and detoxification systems.
